Weatherization Application

You are about to apply to KCHA's Weatherization program. As part of this application, you must submit verification of all household income for the last three full months: , , and . Depending on your situation, these documents may include:

  • Copies of pay stubs
  • Letters from the Social Security Administration
  • Unemployment printouts from the Employment Security Department
  • Copies of self-employment records or an annual tax return
  • Direct deposit records of pension or social security
  • Child support documentation
  • Alimony documentation

You must also provide:

  • Copies of social security cards or Medicare cards showing both the name and the social security numbers of all household members over 1 year of age. Green cards, I-94 forms, etc. can be used in place of social security cards.
  • Copies of heating & electric utility bills
  • Documentation that verifies ownership of your home – either by you or your landlord

All required documentation must be provided with your application to avoid processing delays. You will be asked to mail or email it to KCHA immediately after. (The address will be provided during the application process.)

Please note: submitted applications are valid for one year; if all material isn't submitted by that time, you must fill out a new application. Please also note that KCHA may request additional documentation at a later date.
